Yes it is a written policy however in the early days enforcement was whatever the security guard felt was appropriate dress not necessarily what was written.
The policy applies to all guests of hotel guests. There was an article in
AM Costa Rica sometime ago describing how two Ticas were denied admittance to the News Cafe at lunchtime. One of the women was the newspapers attorney. (Hold the jokes about attorneys being whores
