If your $29 exit tax is included in your ticket (it's probably listed separately as a $27 tax and $2 tax), you won't have to pay the tax, but you will still need to fill out the form that was on the back of the exit tax that we used to have to pay for.

Thanks Costareeker, who sent the blank form to me, I uploaded it to my site.

You can download it and have it filled out beforehand: http://www.onlineorange99.com/crt/exit.jpg

Obviously, print it in color so it's yellow, and cut it to size. They won't accept an 8.5 x 11 sheet. Also, I was just told by a VRT member that American Airlines gives out a wheet with the AA logo on it. So it may not work for all.

*If your tax is not included in the ticket, you still have to buy it and fill out the back.

The line items under Taxes & Fees on my UA ticket from this past week were as follows:

Costa Rica Baggage Inspection Fee: $2.00
Intl. Boarding Tax: $27.00

As of 10/2 SW does not have these taxes included, nor does Delta as far as I know.

Delta does, listed differently.
These were purchased late July early August for our flight next week.

Listed as:
Sierra Leone - Sierra Leone-Immigration fee (A1) $2.00 USD
B1 Tax $27.00 USD

Now that I have looked at the form Orange uploaded, that is the same form I filled out when I came to Costa Rica. I think most of the flight attendants will give you an extra form if you ask for it. I asked the front desk person at the hotel to print it, and he printed it the size of an 8 1/2 X 11 sheet of paper. I don't know if the airline will accept it, but I will have all the info there so I'll only have to copy it.

You are confusing this form with the arrival form you get on the flight when you travel to CR. This is different.

When you print it out, print it in color so it's yellow (print it before you go to CR- the hotels don't have color printers, and even if they do, they won't waste color ink on you anyway, LOL), and cut it down to the size that it is. I wrongly assumed that everybody would think to do that. My bad there.

Odd I didn't fill out or give them anything in early August
Sure it must have either been an oversight or the rules changed just like the amount seems to continually do
