Man I can vouch for that. I was involved with one there for a long, long time and spent many days in Perez Zeledon or PZ which is what ticos are more likely to call San Isidro, but both are correct. Also, there are 2 San Isidro's in CR--the one I think you are referring to is formally known as San Isidro de El General which is the one known for their babes.
Alot, alot of German blood, alot of German first names, Heidi, Ingrid etc, alot of blonds/light hair, blue/green eyed among the equally gorgeous traditional latina lookers. No clue how the Germans got there other then maybe WWII influx in Central y South America? But one knock out after another. So odd you would think that's crazy, but it's true.
Very nice town with 2 major hospitals, only 45 minutes downhill 2300 feet, my favorite road in CR for incredible views, to the Playa Dominical area, so an hour and a half to Manuel Antonio, great high altitude climate, cheap property with incredible views, all the commercial necessites readily available, low crime rate, significant amount of ex-pats for all those reasons, but for hookers, not so much. Very Catholic and despite its size, a small town atmosphere--if they don't know her, they know her cousin kind of a place.
It's roughly 3-4 hs away from SJ depending on the mountain fog via the 2nd most dangerous CR Highway thru the mountains (per capita deaths--I think I read that there were app 20 in 2013). So, what would be the word to describe PZ/SI--parochial, provincial, a little isolated, protected? The genes that control eye candy certainly have been.
But, I knew of only a few bars where p4p was available and the selection was horrible and both were really dumpy. This P4P info is somewhat dated, but the rest of what I have stated I can't imagine has changed much, maybe property values.
