SJO's runway is just shy of 2 miles (1.87) at almost 9900 feet long. It is built on a slope, but not an unusually steep one. It's not unusual to have bit of slope for airports built near mountainous terrain. The east end of the runway is a little over 100 feet higher elevation than the west end. Airports in the USA like Burbank and Las Vegas have similar slopes. There is a standard ILS (category 1) used when landing to the east. There is no instrument approach straight in when landing west because of the terrain to the east.
I'm not a pilot so I can't speak first hand, but I've gotten the impression from the pilots I fly with that they don't mind the slopes too much. What they hate is when there is a "hump" or rise mid way down the runway. The one in Puerto Plata, DR is like that. It can really reduce braking effectiveness and even toss the plane back in the air if you are carrying speed or not dumping lift. Pretty scary taking off that runway in DR when there's like a 50 foot drop to the ocean right at the Eastern end of it.
When I left SJ last visit, we took off on 25 and did a big loop around to the right as we departed. The weather was patching thunderstorms all around but just as we went over Poas Volcano it was crystal clear. Perfect view to end my last trip
