I can only speak again, from personal experience. I cut my finger on some glass pretty deep before I was a resident and went to a Caja clinic. They came in, cleaned it all up with stitches, gave me a shot and some antibiotics and sent me on my way, no money ask for and no bill. When I subscribed to the Caja and all my subsequent visits, nothing has ever been said about any bill or any money I owed for that visit...
The problems at the Quepos hospital sounds like someone had/has their hand in the till...not an uncommon thing here in CR. Know of one CRT member who paid a hotel bill in cash. The employee pocketed the cash and ran his credit card (no signature) for the bill. He, of course, contacted the hotel who had, by that time, fired the employee. They refunded the charge on his credit card which was good of them as he didn't have to take action through his bank.
