In my experience in Latin America, a novio/novia is simply someone that you are dating with a romantic interest. If it has become a serious, monogamous relationship, then the women becomes the man's "mujer" and the man becomes the woman's "marido", and you don't use the term novio any more. However, if the couple is engaged to be married, then they are "comprometidos".

I agree with Chi_trekker that working girls are either amantes or queridas, because that implies that it is a relationship built on sex and convenience rather than romance.
