I have tried a couple in San Pedro, not that good, can't remember the names. But Okami in Barrio California was very good, I have returned twice. Expreso 2221 0725.

Matsuri in Plaza Antares, San Pedro.
Ichiban, various locations including San Pedro, Rohrmoser and Heredia.

crowne plaza has a pretty nice one upstairs w benihana type grills as well.
i have taken a lot of guys there and they all liked it. i think it is called fuji.
a little on the pricey side depending on what you order but if two of us go and eat a lot of fish it ends up being less than 100 typically w drinks.
nothing too exotic but everythings fresh and the chef is japanese. :D

From the above: There are 5 Restaurantes Matsuri: http://www.restaurantematsuri.com/matsuri/restaurantes/
Also 5 Ichibans: http://www.ichibanrestaurante.com/es/index.html << this site takes forever to load via Search--perhaps they'd trade a meal or two for help?
Hotel restaurants: http://www.costaricabeauty.com/viewprof ... pon%C3%A9s << in the Barcelo SJ Palacio
https://www.facebook.com/FujiRestaurant.CR << in the Crowne Plaza Corobici...used the Facebook page, cuz the native site is beautiful but Ph. # and hours on the home page are unreadable

I get it that the OP was looking for personal recommendations but try the Usual Suspects (TripAdvisor Fodor's, etc); also TicoTimes might be a source...perhaps biased towards advertisers but still..:

Not sure if anyone mentioned this place. I can't even remember the name. But it is in Cariari right off the pista near the Fresh Market. The sushi there was excellent.

Info about Saretto was sketchy (zip FB presence) and often wrong (Lonely Planet thinks it's a restaurant).
location and Phone/fax: http://www.tuugo.co.cr/Companies/sarett ... 3100023453
It was bought last year by a big chain, so it will have more of a presence in the future.

Anyone remember the sushi place across the street from The Morazán 2004 or close to it in terms of the year ? They were pretty good maybe ahead of the time and lasted a year or so. Like Rico's across from the Del Rey before the Monkey Bar. A lot of so called experts on this board have no clue what I am talking about sort of like mentioning Arte Sauna or Park Hotel . Maybe before Rico's was Rogers Bar. When New York Bar was in the now Del Rey parking lot. Lots of fond memories of the past.

A bit off topic, but if you are in Jaco I definitely recommend Tsunami Sushi. Probably some of the freshest sashimi I have ever eaten.
