The ID cards, called cedulas, are carried by Ticas who are 18 or over. It is an ID card that is issued to all CR citizens when they reach 18. It is not, in any way, a health card.
It is possible that some escort services would insist on their girls having periodic check up's and blood tests but it is not a requirement imposed by the government. You would have to ask the service their policy and hope they are truthful.
