Just use the website
https://salud.go.cr/ . You can take a photo/Screenshot of it. Can also print it. Takes 5 to 10 minutes to fill it out. No big deal.
That's the existing one you need to complete before entering the country. What we are talking about here is a new one that everyone will need if they want to enter bars, restaurants, etc. Not the same.
The new QR code will also be required to enter hotels. It is quasi-optional on December 1, but mandatory on January 8. After that date, hotels are REQUIRED to refuse you entry. If they fail to do this, the government says it will fine and/or close the place down.
Bear in mind these important dates
December 1 - The vaccine pass to come into effect for
EVERY human being in Costa Rica (citizen, resident, tourist). Hotels, restaurants, bars, etc. can delay requiring it if they keep current restricted occupancy limits. (19 days away)
January 8 - The vaccine pass MUST be checked in order to enter any hotel, restaurant, bar, etc.
YOU must show the pass either on your phone or a printed copy. The
VENUE must scan the pass to validate. At this moment, the app to read the code is only available on the HUAWEI app store, not Apple or Android. (57 days away)
February 6 - The TICOs vote for a new election. They will elect a new president, two vice presidents, and 100% of the legislative deputies (57). If no presidential candidate gets at least 40% of the vote, a run off election is held April 3. There are 26 different candidates and the current president cannot run and the leading candidate is from a different political party. (
The current president says that this will not change anything)
May 9 - The new president takes office, the current president leaves. (
The current president says this will not change anything).
