I printed out my Confirmation of Coverage page and luckily noticed a few things missing from the page, name, passport #, and effective date. Policy number was printed. I tried a few things with no avail. I checked ink levels, printed in color and black and white, still not printing a few characters. After a couple of days I decided to change ink cartridges even though old cartridges still had ink when I noticed I had installed HP knockoff ink cartridges. I installed HP ink cartridges and problem solved. All characters are present. The only thing I can figure out is that their is some security item with the printout that after market ink cartridges couldn't read. Just a heads up to look closely at your documents. With all the extraneous info on the sheet make sure the bread and butter is printed. I was printing on a Hewlett Packard inkjet 3830.
