I am also retired military, live here and have Tricare for Life. Clinica Biblica and CIMA both take TFL. Do a search for TriCare for Life providers in San Jose, Costa Rica and that should show both. There are also a few doctors that take it.
Some doctors file for you and charge you the 25% (up to a max of $3,000 annually) while others give you the paperwork to file (you pay up front and then file yourself with Tricare).
You can contact Tricare International SOS Customer Service for Costa Rica at +1 215 942 8393 or tollfree 0800 044011. I have found them to be very helpful with general questions.
By the way, you are covered as a tourist here or resident.
